For each Sinai-Grace Heart Walking club, the club coordinator should complete and submit a club roster.  This form (which includes the club name, coordinator contact information, and names of the club members) helps ensure that each club member receives a Heart Walking Club membership card.  Show your card at our free blood pressure screening and weight monitoring sites, and to take advantage of other club member benefits.
